Hazard Reporting Form

To report traffic signals which are unresponsive to cyclists please use our unresponsive signals form here.

Thank you for taking the time to be a part of Sonoma County Bicycle Coalition's efforts to make Sonoma County a great place to ride!

Use this form to report conditions in Sonoma County that are hazardous to cyclists on:
1) Roads (with or without bike lanes) or

2) Paved (class I) paths

Please only use this form for the sort of road hazard that a public works department can reasonably be expected to fix such as potholes or pavement cracks, trees or debris in the road or shoulder, vegetation overgrowth which blocks the road or shoulder, or hazardous drainage grates or railroad crossings.

One of our trained volunteers will pass your report on to the appropriate jurisdiction. Please take the time to make your report accurate. We cannot pass on a report of a hazard unless the location and hazard are both described clearly.

We ask for your name and contact information in case we need to contact you for more information about the hazard. Also, we will let you know about any response we get from the responsible jurisdiction.

This form should NOT be used to report emergencies, please contact law enforcement at 911, and in the case of downed power lines PG&E at 1-800-743-5000. While we will do our best to get your report to the responsible agency and many of these agencies do their best to follow up in a timely manner, please understand that filling out this form is no guarantee that the hazard will be fixed.
